Working together with our community partners, including local health departments and other hospitals, we are focused on disease prevention, promoting access to quality healthcare services, and improving the overall quality of life for our communities. We offer many community-based outreach programs for the vulnerable and at-risk residents in the communities we serve. All of our services are provided in a caring and compassionate manner. These programs offer a variety of services to individuals, including primary care and dental care, referrals, education, nutrition counseling, prenatal services, job training, and housing. We are participating in a national initiative - Partnership for Patients (PFP) - aimed at improving the quality of care and making healthcare safer by reducing hospital-acquired conditions and re-admissions. NYS Partnership for Patients was recognized as one of the nation's highest-performing programs and was highlighted by a U.S. Department of Health and Human Services report in 2014.
