We offer a wide range of senior care programs and services, including geriatric medicine, home care, senior living facilities, skilled nursing homes, and the region's only PACE program, aimed at keeping seniors healthy and independent at home for as long as possible.
Age-Friendly Health Care
Our hospitals, Long-Term Care facilities, and ElderONE program have been designated "Age-Friendly Health Systems" by the John A. Hartford Foundation (JAHF) and the Institute for Healthcare Improvement (IHI). An Age-Friendly Health System is one that follows an essential set of evidence-based practices that lead to an overall improvement in patient experience and outcomes for older adults. 
Geriatric Surgical Program
Focused on recognizing the unique physiological, social, and treatment goals of our aging surgical patients, our Geriatric Surgical Program is the first of its find in the United States. Offered at Unity Hospital, Newark-Wayne Community Hospital, and Clifton Springs Hospital, this program provides patients with personalized surgical plans created to meet their needs and ideal outcomes.
