Rochester Regional offers five skilled nursing homes to provide long-term care, along with specialized programs for short-term rehabilitation, dementia care, neurobehavioral care, and ventilator care. 

Rochester Regional has a variety of home-based services to help you or your loved one continue to live safely and independently at home such as meal delivery, companion services, home health aides and skilled nursing visits. We also offer a personal emergency response system and medication monitoring system.

At the Memory Center at Unity, our highly specialized team provides comprehensive and compassionate care for neurodegenerative conditions affecting older adults and the aging nervous system. If you or your family are concerned about your memory or thinking skills, it may be time to visit the Memory Center.

Otherwise known as a Program of All-Inclusive Care for the Elderly (PACE), ElderONE is the only program of its kind in our region. Through ElderONE, seniors can receive all the medical, social, and emotional care they need, while still being able to live at home. 

Our geriatric primary care team provides primary medical care tailored to senior patients. With expertise in healthy aging, we work alongside you and your family to ensure you receive the care that best meets your needs.
