Diabetes Education and Nutrition Services

Learn more about living well with Diabetes or other Chronic Diseases 

Our Diabetes and Endocrinology teams have provided quality education programs taught by experienced Diabetes Care and Education Specialist for over a decade. Many patients choose to work with their own physicians to manage their disease and come to the Rochester Regional Health’s Endocrine-Diabetes Education program for additional educational services.

For those who have diabetes, we offer individual and group education along with a library of books, videos, Web sites and applications (apps) to help with diabetes management, and other resources. Our education programs do not require a referral to an endocrinologist; they can be accessed individually based on your needs. 

Our Diabetes Education program, recognized by the Association of Diabetes Care and Education Specialists, include individual consultations and group classes with a Nurse or Registered Dietitian, Certified Diabetes Care and Education Specialist. We offer a patient-focused holistic approach both to preventing and managing diabetes including gestational diabetes. Information is available on everything from tracking carbohydrates to using insulin.

Our Registered Dietitians are also here to help you with other chronic disease and nutrition concerns including Pre- diabetes, Celiac disease, Cholesterol issues, hypertension, weight loss and more.

Educational programs 

Individual appointments with our registered Dietitian and Registered Nurse Diabetes Care and Education Specialist are available, and may include follow-up sessions.

Our Certified Diabetes Care and Education Specialist work with you on Nutrition, Insulin and other injectable instruction, Glucose meter instruction, Blood sugar monitoring, Insulin pump and continuous Glucose monitoring instruction and other areas of diabetes management.

Our diabetes education programs help you create a diabetes self-management plan that fits your lifestyle. Certified Diabetes Care and Education Specialist show you how to monitor diabetes, select and prepare tasty foods, deal with stress and illness, and make physical activity an enjoyable part of your routine. In short, we’re here to help turn a difficult diagnosis into a positive life change.

These group sessions (6 hours total- 2 hour/week session over 3 consecutive weeks)) provide all the information needed to stay healthy with diabetes. Classes include medical management of diabetes, preventing complications, and exercise with diabetes.

Planning on going on a pump or Continuous Glucose Monitor (CGM)?  This comprehensive instruction provides information to help you be confident and successful with your pump therapy or CGM system. For those currently on pumps or CGM system, evaluation and assistance with fine-tuning is available.

Description: Food-based weight loss and ongoing support that is designed to teach you the skills for both successful weight loss and maintenance.  Visits are one-on-one and taught by a registered Dietitian with expertise in nutrition, exercise and lifestyle behavioral change.

Cost:  $55 for 1 hour initial visit and $35 for ½ hour follow up visit
