Connecting You to the Support Services You Need

Health Care Management is a support team of healthcare and service providers, who will help you get the healthcare and services you may need. This free program is available to those enrolled or eligible for Medicaid and is designed to help people who may have behavioral health or chemical dependency needs, or who experience ongoing chronic health issues. 

When you are enrolled in Health Care Management, you will have your own Care Manager, who is with you every step of the way to your life-long health goals.

We can help if you experience any of the following:

• A chemical addiction condition
• A mental health condition
• A chronic physical condition
• Difficulty finding a doctor or getting to your appointments
• Frequent visits to the emergency room
• Unsafe living conditions
