Call To Transfer A Patient
To transfer an adult medical/surgical patient, pediatric patient, or psychiatric patient, call the Rochester Regional Health Transfer Center.
How our transition of care team can assist
- Transferring patients from outpatient to inpatient; skilled nursing and other facilities to ED; ED to ED; or making lateral moves within the hospital network
- Confirming the patient's information is complete in the system for a safer transfer and sharing necessary information and patient records with everyone who'll need it
- Reducing the amount of time it takes to get your patient to the right provider at one of Rochester Regional Health's hospitals
- Making sure the provider seeing your patient knows what you want evaluated, with physician-to-physician hand-off
What to expect of our transition of care team
- Follow standard protocols and processes
- Identify the appropriate provider and admission method (such as ED or direct-admit)
- Contact you to discuss pertinent information that may impact bed assignment, then sharing bed assignment and necessary arrival information with you when ready
- Obtain your direct contact information so the admitting physician or ED providers can give you updates after evaluation or care
- Continually work to improve the referral process and make you aware of new developments
