Admissions

To be considered for admission to the McCormick Transitional Care Center skilled nursing facility, an application must be completed and returned to our long-term care admissions office.

Admissions Information

Amenities at McCormick Transitional Care Center

All rooms at the McCormick Transitional Care Center are private and have a view of the beautiful courtyard. Each room also has a private bath and a flat-screen TV. Meals are served buffet-style in the spacious dining room and smaller lounges are located throughout the building to provide comfortable spaces for conversation and relaxation. Computer and wireless internet access is available.

A Pandemic Emergency Plan has been developed to enable McCormick Transitional Care Center to effectively mitigate, prepare for, respond to, and recover from a high impact communicable disease outbreak in the community.
