The 40-bed Timothy R. McCormick Transitional Care Center, named after retiring president and CEO of Unity Health System, offers short-term care following hospitalization so patients can recover before returning to their own homes.
Our patient-directed philosophy of care is simple: we're focused on you. Our team will work with you and your hospital medical team to develop a plan of treatment based on your unique recovery needs.
To be considered for admission to the McCormick Transitional Care Center skilled nursing facility, an application must be completed and returned to our long-term care admissions office.

Admissions InformationAll rooms at the McCormick Transitional Care Center are private and have a view of the beautiful courtyard. Each room also has a private bath and a flat-screen TV. Meals are served buffet-style in the spacious dining room and smaller lounges are located throughout the building to provide comfortable spaces for conversation and relaxation. Computer and wireless internet access is available.
